Patient & Carer Charter (not limited to Dementia)

NHS Nottingham and Nottinghamshire are developing a new Patient and Carer Charter to guide individuals on what to expect when someone is discharged from a hospital to another care setting, such as a community hospital or care home on a temporary basis—also known as a “pathway 2” discharge. They are seeking input from carers who have experience with this process to ensure the charter is as helpful as possible.
